The Patient

The patient (or client) is the person receiving the health maintenance care as determined by the Medicaid case manager. In many cases, this is the child of the person providing the care, called an attendant.

Medicaid Case Manager

The Medicaid case manager is the first point of contact for families interested in pursuing IHSS services. The case manager can help determine available services, the best service delivery option, the plan of care, and how many service hours are needed per week. Once those are determined, the case manager will help you identify local IHSS agencies.

Attendant

Attendant

The attendant is the person delivering the health maintenance activities to the patient. Typically, the attendant is a family member, such as the mother or father of the patient, or another close relative chosen by the family. This person is employed through the IHSS agency, in this case Circle of Care Colorado.

Authorized Representative

The authorized representative (AR) is an individual designated by the parent(s) or legal guardian of the pediatric patient. The parent(s) or guardian must have known this person for two years and trust this person to help manage the IHSS services. In many cases, the AR is the father, aunt, uncle, grandmother or a trusted family friend of the patient.

Circle of Care Colorado is the IHSS agency in this scenario. We are responsible for determining employment eligibility of the attendant, onboarding the attendant, providing 24/7 support, ensuring necessary skills can be performed and providing payroll services.
